Pillar 2: Dementia Support and CaregiverEmpowerment Program

Program Overview

 The Dementia Support and Caregiver Empowerment Program is designed to improve the quality of life forindividuals living with dementia while providing education, support, and resources for caregivers and familymembers. Through therapeutic activities, educational workshops, and community engagement, theprogram seeks to reduce isolation, enhance caregiver confidence, and promote meaningful connections forfamilies affected by dementia.

Target Population

  •  Individuals living with dementia and related cognitive conditions
  • Family members of dementia patients
  • Professional and non-professional caregivers
  • Community members seeking education about dementia care

Program Goals

  1.  Improve emotional well-being and engagement among individuals living with dementia.
  2. Provide caregivers with practical tools and knowledge to enhance caregiving effectiveness.
  3. Increase community awareness and understanding of dementia-related conditions.
  4. Reduce caregiver stress and strengthen family support systems.
  5. Connect participants with ongoing resources and support services.

Program Activities

 

Music Memory Therapy 

  • Music-based therapeutic sessions designed to stimulate memory, engagement, and emotionalconnection.
  • Interactive group activities that encourage participation and socialization.
  • Personalized music experiences that promote comfort and positive memories. 

 

Caregiver Education Classes

  • Training on dementia care best practices.
  • Workshops on communication techniques, behavior management, and daily care strategies.
  • Guidance on caregiver self-care and stress management.

 

Family Education and Support

  • Educational seminars on dementia progression and care planning.
  • Family support groups and peer networking opportunities.
  • Information sessions focused on maintaining meaningful relationships throughout the caregiving journey.

 

Resource Navigation and Community Support 

  • Referrals to healthcare providers, support agencies, and community resources.
  • Information on financial assistance, respite care, and long-term planning.
  • Community outreach and awareness events.
